The Primary Care system is undergoing a transformative journey, aiming to overcome fragmentation and uncoordinated care that contribute to inequities and unnecessary strain on emergency departments and hospitals. By fostering interprofessional collaboration, embracing innovative workforce strategies, and leveraging data-driven decision-making, we can address these challenges effectively. This talk delves into how an action framework can seamlessly integrate research, quality improvement, and care delivery to accelerate evidence-informed solutions. By prioritizing co-design with patients, caregivers, and clinicians, we can create a more responsive and equitable primary care system, ensuring better health outcomes for all.

Kim Simmonds, PhD, assumed the role of Primary Care Alberta Chief Executive Officer on Nov 18, 2024.
